To enter the Delta Golf Open:

Complete in full the Tournament Entry Form

List each name in your group. Dept. # (VIP for Vendors/Suppliers, RET for Retirees), City, Employee number and your average score for 18 holes. Check the seniors flight if you are over 60 years of age, or the Herman Manley Super Seniors Flight if you are 68 years of age, and wish to compete in either of these two flights.

Entry fee. Make out a check for $25.00 made payable to the Delta Golf Committee for each name. 

NOTE: The entry fee for Vendors/Suppliers is $300 per person includes Hole Sign!.

Tee Times-As previously stated, we will again utilize tee times for our foursomes this year. Please check on the entry form the tee time you prefer, AM Sat/Sun, PM Fri/Sat or PM Sat/Sun. Please note your 1st, 2nd and 3rd choice. We will try to accommodate everyone; however, each block of tee times is limited. Tee times are assigned on a first come, first served basis
